Implementation:

 

21 – Are there emergency moratoriums against any new/similar project’s/products safety? (Yes = blue)

22- Have technical/operational problems added more financial loss? (Yes = blue)

23 – Are parts procured to ensure interoperability, such as:  (Yes – good)

24 – Has/will the customer receive a brief tutorial regarding how to master the product/service more quickly? (Yes = green)

25 – Do project trainers observe/ride with new operators to determine what the project team is/may have gotten wrong? (Yes = green)

26 – Will the installation of the new system severely disrupt the customer’s current operations? (Yes = red)

27- Is the intended user literate? (No = red)

 

Quality:

7 – Do QA/technicians know the following about the problem’s root cause (Yes=good):

  1. When did it occur?
  2. Where did it occur?
  3. Why it occurred?
  4. How it occurred?
  5. Is this a new problem/issue?

8 – Do test results produce excessive:

  1. False positives
  2. False negatives
  3. Inconclusive results

 

9- Do team/technicians/managers have the training/skills to correctly interpret the test results?

10- Have all the systems been fully tested as an integrated/interoperable system as they will operate under regular/abnormal conditions? (Yes = good)

11- Does testing show continuing/repeated/unreliable results/unresolved safety issues? (No= green)

12- Does the product/service have functions that nobody understands what they do? (Yes = red)

13- Is QA doing enough and appropriate testing to ensure product’s success? (No = red)
