1 – Is the cost/ schedule performance greater than 10%? (NOTE: Green = 0 to 10%;  Blue = 11 to 24%;  Red = 25% or more)
2 – Is the schedule performance index/ cost performance index less than .9? (See Cost Control Equations in Section 7)
3 – Is the project technical lead present on this project?
4 – Is project manager unqualified or the position vacant?
5 – Does feature creep increase by more than 2% each month?
6 – What is the size of this project?  (large = red; medium = blue; small = green)
7 – Is an ongoing contract dispute occurring on this project?
8 – Is project’s duration greater than 18 months? (NOTE:  Yes = Red)
9 –Does a project plan exist?
10-Is an acceptance plan in-place?
11-Is more than 75% of the contingency resources gone before the project is 75% complete?
12–Is was contract legally reviewed (do throughout the project)?  
13-Is the project champion position vacant or perceived as weak?  
14-Are more than 15% of the project’s issues still open 
15-Is the probability of failure greater than 25%  
16-Is the project team in turmoil?  (See Section 2:  Project Trouble Signs)
17-Are 30% or more of the key positions vacant? (Yes = bad)
18-Are 70% or more of the key action commitments completed on schedule or progressing adequately? (Yes = good)
19-Is SM pleased with the progress being made? Is progress occurring at the right speed? (Yes = good)
20-Has the project sponsor left the firm?
