MORALE/COMMUNICATION:

1 – Does the project team perceive that management deals with them fairly?
2 – Does the team exhibit pride, elan or espirit in meetings and to outsiders?
3 – Does/will the team receive a reward if key performance targets are met?
4 – Do team members trust each other’s integrity/ competence?
5 – Do team members trust management?
6 – Do team members trust support organizations/vendors/contractors?
7 – Do project team members clearly understand the project’s strategy? Mission? Plan?
8 – Do individual team members enthusiastically perform their duties?
9 – Does the team perceive that it is overworked?
10 – Does good internal communication (can talk freely) exist within the team?
11 – Do team members have the required authority to do their jobs?
12 – Do individual team members appear to be unduly challenged by their jobs?
13 – Do individual team members creatively solve project problems?
14 – Does the team react well to criticism?
15 – Are team members made to feel like owners?
16 – Are work areas neat and well organized?
17 – Does the project experience a higher than usual sick/absence rate?
18 – Does the team experience a frequent rate of personnel turnover?
19 – Are project team recommendations accepted?
20 – Are project team recommendations acted upon?
21 – Do frequent conflicts occur between the project team and functional areas?
22 – Does the project team have good relations with suppliers, consultants and vendors?
23 – Does the team understand the data that they have collected?
24 – Does a system exist to determine which team members merit incentive pay increases?
25 – Does the team buy into management’s vision for this project?
26 – Can team members anonymously report problems (to management)?
27 – Team has forward looking view and are able to predict what is going to happen both in their own functions and across the table?
28 – Do team members regularly explain their projects and give each other feedback & opposing points of view? Are opposing viewpoints respected?
29 – Do team members have the ability to perform any work, independently?
30 – Do team members feel they receive proper recognition for a job well done?
31 – Team members feel a sense of fun while working on the project?
32 – Is project environment too machismo oriented? (Yes = blue)
33 – Can team members execute their individual initiative? (Yes = green)
34 – Is team empowered to interview/ hire project candidates? (Yes = blue)
35 – Does team know the project’s chain of command (who runs what?)? (No = bad)
36 – Are team members comfortable enough with each other that they involuntarily: Stay late, toss around ideas, play board games, eat informal meals together? (Yes = good)
37 – Do team members feel that this project provides them with the opportunity to contribute to something bigger than themselves? (Yes = good)
38 – Can team members openly discuss how they are feeling about personal/ professional issues? (Yes = good)
39 – Do subject matter experts periodically revisit the state of scientific knowledge in their chosen field? (Yes = good)

PLANNING & DECISION MAKING:
1 – Are project meetings well managed (agenda, timed, purposeful, right people)?
2 – Does the team have an effective filing/recordkeeping system? (e.g. minutes, memos, letters, newsletters, specifications, contract documents, status reports, etc.)
3 – Do scope changes frequently (due to things added as an afterthought) occur on the project?
4 – Does the project team frequently accomplish the project’s planned objectives?
5 – Does the team know the project’s constraints?
6 – Are resource conflicts severe on this project?
7 – Does the team understand which are the critical success factors (CSFs)?
8 – Does the team have the power to make decisions that affect their work?
9 – Must work performed first be recorded on paper, before being entered into a computer system? (NOTE:  Possibilities for delay, error and fraud)
10 – Have measures that have been implemented to-date, brought the project under control? (No = blue)
11 – Can/ has SM/ PM gauge the full scope of the project? (No = blue)

12 – Does PM make decisions based on actual data – collected from the bottom-up (as opposed to top-down)? (Yes = good)
